Horizons provides an array of residential services based on each individual’s needs. Some individuals are served in homes with 24-hour care. To the extent possible, these individuals participate in daily routines such as cooking, cleaning, laundry, yard work, etc. Some clients need comprehensive and constant support while others need only minimal assistance to manage their personal care. Horizons also provides apartments for those individuals who can live independently with support. Counselors are available to assist with life skills such as grocery shopping, meal preparation, household chores, budgeting and bill paying. The amount of service provided varies by individual. Currently, these services are only available in Moffat and Routt Counties.
