Horizon's Day Program includes independent living skills such as money management, continuing education, health management, recreation and social skills. It may also include activities to enhance fine or gross motor abilities, exercise to enhance overall health, and strategies to improve coping skills. These services are currently available in Moffat and Routt Counties. |

Horizons provides an array of residential services based on each individual’s needs. Some individuals are served in homes with 24-hour care. To the extent possible, these individuals participate in daily routines such as cooking, cleaning, laundry, yard work, etc. Some clients need comprehensive and constant support while others need only minimal assistance to manage their personal care. Horizons also provides apartments for those individuals who can live independently with support. Counselors are available to assist with life skills such as grocery shopping, meal preparation, household chores, budgeting and bill paying. The amount of service provided varies by individual. Currently, these services are only available in Moffat and Routt Counties. |

Supported Living Services (SLS) are used to supplement already available supports such as a person living independently with limited supports or with their family. SLS can provide a variety of services such as personal care (eating, bathing and dressing) or homemaking needs, employment or other day type services, or accessing their community. We help with decision-making, assistive technology, home modification, professional therapies, transportation, and twenty-four hour emergency assistance. Supported Living Services are not intended to meet all of a person’s needs. Service needs and the level of support are prioritized within the person’s Individualized Plan. The individual is responsible for their living arrangements, which can include living with family or in their own home. It is possible to have a roommate in this program. Many clients of Horizons work in the community, either for an employer or on a work crew supervised by a Horizons Job Coach. When not working or for those who work part time, clients are involved in Day Program (where we work on computer, math, reading, writing, cooking and life skills) and community activities, concerts, picnics, hikes, library visits, art classes, physical and recreational programs, and more. |
